Pathway to Wellness Coalition

The Smith County Memorial Hospital Pathway to Wellness Coalition is a community-focused partnership dedicated to improving the health and well-being of residents throughout Smith County and surrounding rural communities. Led by Smith County Memorial Hospital, the coalition brings together healthcare providers, community organizations, local foundations, schools, businesses, and community members to promote preventive health, physical activity, healthy lifestyles, and improved access to wellness resources. Through initiatives such as the Pathway to Wellness, outdoor fitness opportunities, educational activities, and community wellness events, the coalition seeks to create sustainable opportunities for individuals and families to make healthier choices and remain active throughout their lives. By combining the resources and expertise of local partners, the Pathway to Wellness Coalition is working to strengthen community connections, address barriers to healthy living, and build a healthier, more vibrant Smith County for current and future generations.
