Envelope Link Facebook Contact: Susan Jagerson About the Coalition: The Access to Healthy Food Coalition was formed in 2015 through a National WIC Association grant and officially incorporated as a non-profit organization LWGC in 2016 as a result of a Community Health Needs Assessment (CHNA) showing the need for a county-wide coordinated action among health and wellness stakeholders to address food insecurity and other health issues. The Coalition is a diverse group of volunteers from local organizations and community members with a passion for making Geary County a healthier place to live. The Coalition includes members from target populations. Live Well Geary County is a Phase 1 and Phase 2 Pathways to Healthy Kansas grant recipient. The Live Well Geary County coalition exists to inspire and advance healthy lifestyles in Geary County. The purpose of the coalition is to: develop short and long-term community goals to improve community health, align and coordinate member efforts so that they are mutually reinforcing, inform the public about issues related to the health of the community, and mobilize public support for policy, systems, and environmental changes that support improved community health and wellness.
